600 Bilateral Upper Limb Compartment Syndrome After Massive Postpartum Haemorrhage

Abstract Case Summary Upper limb compartment syndrome (ULCS) is an uncommon limb-threatening emergency occurring as a result of increased interstitial pressure within osteofascial compartments. The overwhelming majority causes occur in one limb, with frequent including fractures, reperfusion injury and burns. We report the unusual case bilateral upper CS previously well 35-year-old female secondary to massive resuscitation following postpartum haemorrhage. patient received 7 litres blood products 4 crystalloid over period 8 hours. One hour postoperatively both hands became painful swollen. Symptoms worsened hour: forearms were tense palpate demonstrable pain on passive extension wrists digits left hand, capillary refill all was brisk. Creatinine kinase measured at 4,600U/L forearm left-hand fasciotomies performed. presence bulging muscles hand confirmed diagnosis syndrome, perfusion restored after release later underwent split thickness skin grafting fasciotomy wounds. After 5 weeks therapy, she regained good function hands. Bilateral ULCS extremely but has been reported resuscitation, particularly trauma patients, we suspect that this cause our patient.
